Case 12 - 8-year-old male presents with a clinical concern for bilateral hip dysplasia

<div>HISTORY:&#160;This 8-year-old male presents with a clinical concern for bilateral hip dysplasia.PRIMARY FINDING:&#160;Developmental dysplasia.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Exaggerated anteversion of the left hip. Shallow left acetabulum with dysplastic femoral head and neck. The left hip is completely dislocated. Elongation of the external rotator muscles appear obliquely in the coronal plane (obturator externus especially).Fixation hardware noted along the anterior neck of the left hip likely sequela of previous rotational osteotomy.&#160;1-2+ left hip joint effusion.&#160;Left and right ligamentum teres intact. No labral tear or paralabral cyst. Tiny millimeter sized pre-acetabular right iliopsoas bursal cyst.&#160;Volume loss, atrophy of the left gluteus medius and minimus without insertional tendinosis along the trochanter and neck of the involved femur.&#160;No advanced joint space chondromalacia of the affected left hip; however it is difficult to appreciate any labral remnant. No contralateral right labral tear however the right labrum articulates with the superior femoral neck as sequela of coxa vera.Bilateral dysplastic hips.&#160;CONCLUSION:&#160;1. Chronically dislocated left femoral head. Dysplastic hips. Left hip joint effusion associated with shallow acetabulum. Left proximal femur also demonstrates exaggerated anterior version. Associated signal alteration and atrophy of the abductor musculature and left gluteus maximus versus the more normal contralateral side.&#160;2. Right hip coxa varum. Component of broadening of the contralateral right femoral head-neck angle supporting bilateral hip dysplasia.&#160;3. Small millimeter sized pseudocyst involving the pre-acetabular region of the left iliopsoas tendon.&#160;4. Plain film correlation would be useful.</div>
