Learn to master subtle stroke cases on CT and MRI! Gain experience detecting strokes on CT by reviewing the 25 cases of subtle strokes presented in this module. The purpose is not to comprehensively explain the role of imaging in the evaluation of acute stroke or to teach a junior physician how to approach a CT scan of the brain. Instead, by reviewing these difficult cases, the reader will become better at detecting the subtle CT findings of acute stroke. Some of these cases were initially interpreted correctly. Many were not.

Case Discussion:
1. Given the clinical history of right-sided symptoms within a learning module dedicated to subtle strokes, proper windowing to accentuate the cortex, and a methodical slice-by-slice approach, a subtle left parietal hypodensity should raise suspicion for an acute infarct. This region is particularly well suited for assessment on the sagittal reformat.

Imaging follow up MRI (Scan 2)
Subsequent brain MRI examination reveals an acute left parietal infarct (high DWI signal, low ADC signal, abnormal FLAIR signal) within the left middle cerebral artery distal M2 inferior division vascular territory.

Primary teaching point:
Utilization of provided clinical history, systematic windowing, and high clinical suspicion should raise sensitivity and accuracy for detecting acute infarcts on unenhanced CT examination.
