HISTORY:
This 77-year-old male presents with severe headaches and blurred vision for six to eight months.

Intracranial mass.

FINDINGS:
Hydrocephalus ex vacuo with extensive supratentorial atrophy and peri-ependymitis granularis and/or leukoariosis.
Posterior fossa / cerebellar atrophy.
No signs of diffusion restriction.
Cavum septum pellucidum et vergae.
No macroinfarction in the visual pathway is noted.
No signs of intracranial enhancement or neoplasm are noted.
IMPRESSION:
Pattern of extensive neurodegenerative atrophy with ex vacuo ventricular enlargement and transependymal migration with deep ependymitis granularis but no evidence of macroinfarction or intracranial mass.
