<div>HISTORY:&#160;This 39-year-old male presents with peritrochanteric fracture of right hip, status post fall off bike. Difficult weight bearing.PRIMARY FINDING:&#160;Intertrochanteric femoral fracture.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: A mildly comminuted intertrochanteric fracture with a 2.2cm trochanteric fossa fragment attached to contused obturator externus tendon and torn inferior gemellus. Swelling into the remaining short hip external rotators which have intact attachments. Fragment off the superoposterior facet of greater trochanter remains attached to the swollen superoposterior head of the gluteus medius with an underlying mild gluteus medius muscular strain. No avulsion of the right lesser trochanter. Grade 3 of 4 microtrabecular injury extends to the tensile surface of the femoral neck without a macrofracture. Moderate right hemarthrosis has resulted. 1.5cm posttraumatic subgluteus medius bursitis.&#160;Intermediate grade strain of tensor fasciae latae with nearby 11.4cm degloving injury between the iliotibial band origin and the subcutaneous tissues of the lateral hip. Low grade strain involves the right adductor muscle group.&#160;Femoral head remains intact with intact femoroacetabular articular cartilage. 1cm greater than 50% partial thickness tear involves the anterosuperior labrum at 10 o'clock position. No paralabral pseudocysts.&#160;CONCLUSION:&#160;1. Mildly comminuted, nondisplaced, nonangulated right intertrochanteric femoral fracture.&#160;a. Nondisplaced fracture fragment off the trochanteric fossa is attached to swollen obturator externus with tear of the subjacent inferior gemellus.&#160;b. Nondisplaced fracture fragment off the superoposterior trochanteric facet is attached to the associated gluteus medius bundle which is swollen. Nearby low grade strain of gluteus medius muscle.&#160;c. Grade 3 of 4 microtrabecular injury extends into the intracapsular femoral neck to contact its tensile surface. No intracapsular femoral neck macrofracture.2. 1cm greater than 50% partial thickness tear involves the anterosuperior labrum at 10 o'clock position. No chondroosseous separation or paralabral pseudocyst. No chondral or osteochondral lesion.&#160;3. Small subgluteus medius posttraumatic bursitis with moderate right hip hemarthrosis.4. Intermediate grade strain of tensor fasciae latae with an associated 11.4cm fascial degloving injury along the origin of the iliotibial band, compatible with an acute traumatic Morel-Lavallee lesion.</div>
