<div>HISTORY:&#160;This 4-year-old male with achondroplasia. Presents with history of right hip exaggerated anteversion and subluxation. Surveillance.PRIMARY FINDING:&#160;Bilateral hip dysplasia.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: No pelvic mass. Dysplastic contour irregularity of the right-greater-than-left femoral neck. Portions of the right femoral neck and greater trochanter articulate with the broadened right acetabulum. Small foci of susceptibility artifact or intra-articular air noted. In short axis, whereas previously the right and left hips demonstrated an anterior version relative to the horizontal femoral neck angle, both the right and left femoral head and neck angles are now retroverted. Right acetabular cup is dysplastic and shallow, the left acetabular cup is dysplastic. Small left hip joint effusion.&#160;Nominal thinning of muscle bulk surrounding the right hip joint versus the contralateral left hip. This is also evident in overall muscle bulk of the gluteus maximus, medius and minimus versus the more normal left hip. Right obturator internus is hypoplastic.&#160;CONCLUSION:&#160;1. Sequelae of bilateral hip dysplasia related to underlying achondroplasia. Dysplastic right acetabulum now articulates with articular cartilage in the dorsal femoral neck situated between the cartilaginous greater trochanter and cartilage covering the femoral epiphysis. Coxa vara, dysplastic labrum.2. Ossification center within the left femoral epiphysis is visually larger versus the right side.&#160;3. No mass.</div>
