10b - Answer: 62-year-old male presents with prostate cancer

HISTORY: 

This 62-year-old male presents with prostate cancer.

PRIMARY FINDING: 

PI-RADS V.

FINDINGS:

Prostate measures 4.5cm in transverse dimension, 3.8cm in AP dimension and 5cm in craniocaudal dimension with calculated bullet volume of 56mL. 

Diffuse ill-defined confluent hypointensity of the left peripheral zone with extension to the transitional zones from anterior to posterior and apex to the base with mild restricted diffusion. Possible bladder extension. Focal transcapsular extension at the posterolateral aspect left mid peripheral zone adjacent to the base of the seminal vesicle likely represents transcapsular tumor extension. Ill-defined hypointensity in the anterior apical right peripheral zone, no substantive restricted diffusion. Patchy mild-moderate hypointensity in the right peripheral zones, no substantive restricted diffusion. Neurovascular bundles intact. Mild atrophy of the seminal vesicles. 

No lymphadenopathy in the visualized pelvis. 

Bilateral inguinal hernia with fat. 

CONCLUSION

1. Ill-defined hypointense mass replacing the left peripheral zone with extension to the transitional zones, PI-RADS 5. Focal extracapsular extension at posterolateral corner of mid left peripheral zone. Possible bladder extension.

2. Stage T4 M1.
