14b - Answer: 68-year-old male presents with history of prostate cancer

HISTORY: 

This 68-year-old male presents with history of prostate cancer.

PRIMARY FINDING: 

PI-RADS II.

FINDINGS:

Prostate measures 5.5cm in transverse dimension, 4.7cm in AP dimension and 6cm in craniocaudal dimension with a calculated ellipsoid volume of 81mL. 

Again seen, bilateral wedge-shaped hypointensities in the peripheral zones. No areas of restricted diffusion. Some areas appear more confluently hypointense on T2 sequence. An 8 x 16mm confluent hypodensity at posterior mid peripheral zone at midline at the base of the seminal vesicle with decreased signal intensity on the ADC map, unchanged. No arterial enhancement. Mild hyperplasia is the transitional zones. No confluent hypointense mass. No erased charcoal sign. Capsules intact. Seminal vesicles hypointense and atrophic. Neurovascular bundles unremarkable. 

No lymphadenopathy in the pelvis. 

Urinary bladder diverticula. 

No focal suspicious osseous lesions. 

CONCLUSION

1. Stable MRI. 8 x 16mm hypointense area adjacent to the base of the seminal vesicle stable in size, probably is in the central zone. Wedge-shaped mild-moderate hyperintensities within the peripheral zones with no restricted diffusion, PI-RADS 2. Equivalent of BI-RADS 6 in breast cancer in light of pathology-proven prostate cancer. 

2. Mild benign prostate hyperplasia. 

3. Urinary bladder diverticula suggesting chronic obstruction.
