15b - Answer: 60-year-old male presents with elevated PSA

HISTORY: 

This 60-year-old male presents with elevated PSA.

PRIMARY FINDING: 

PI-RADS II.

FINDINGS:

Prostate measures 6cm in transverse dimension, 4.8cm in AP dimension and 4.8cm in craniocaudal dimension with calculated ellipsoid volume of 72mL.  

Ill-defined mild hypointensity in the peripheral zones with no restricted diffusion. Multiple encapsulated nodules in the transitional zones. No erased charcoal sign. Capsules intact. Neurovascular bundles intact. Right seminal vesicle completely atrophic. 

No lymphadenopathy in the visualized pelvis. No focal suspicious osseous lesions. 

Colonic diverticulosis. 

CONCLUSION

1. No MRI evidence of aggressive prostate carcinoma. Ill-defined mild hypointensity with no restricted diffusion in the peripheral zones, PI-RADS 2. 

2. Benign prostatic hyperplasia.
