HISTORY: 

 This 68-year-old male presents with enlarged prostate and elevated PSA.

PRIMARY FINDING: 

PI-RADS V.

FINDINGS:

The prostate measures 5 x 4 x 6cm with a calculated ellipsoid volume 63mL.

The peripheral zone is atrophic with wedge-shaped mild hypointensity, no restricted diffusion. Encapsulated nodules in the transitional zones. 1.8 x 2 x 3cm solid hypointense mass at the basal right transitional zone crossing midline to the left protruding into the urinary bladder cavity. Mild restricted diffusion and early arterial enhancement. Capsules intact. Neurovascular bundle unremarkable. Seminal vesicles atrophic.

Cellulation of urinary bladder wall.

No lymphadenopathy in the pelvis.

No focal suspicious osseous lesions.

CONCLUSION:

1. 1.8 x 2 x 3cm solid confluent hypointense mass at the basal right transitional zone crossing midline to the left, PI-RADS 5.

2. Benign prostatic hyperplasia.

3. Cellulation of the urinary bladder wall consistent with chronic outflow tract obstruction.
