HISTORY:
This 58-year-old male presents with prostate cancer.
PRIMARY FINDING:
PI-RADS V.
FINDINGS:
Prostate measures 4.5cm in transverse dimension, 3cm in AP dimension and 3.5cm in craniocaudal dimension with calculated bullet volume of 31mL.
2cm in maximum dimension moderate hypointensity lateral aspect of right mid peripheral zone with restricted diffusion. A 4mm hypointensity at mid lateral aspect of left peripheral zone with restricted diffusion. There is early arterial enhancement of these lesions. Hypointensities in all sequences in transitional zones probably represent calcified concretions. A few encapsulated nodule in the transitional zones. An approximately 1 x 1.2cm area of hypointensity at posterior right transitional zone near the base of the seminal vesicle with surrounding ill-defined margin. The capsules are intact. Neurovascular bundle intact. Seminal vesicles unremarkable.
No lymphadenopathy in the visualized pelvis. No focal suspicious osseous lesions.
CONCLUSION:
1. A 2cm in maximum dimension moderate hypointensity at lateral aspect of right mid peripheral zone with restricted diffusion, PI-RADS 5.
2. 4mm hypointensity at lateral aspect of mid left peripheral zone with restricted diffusion, PI-RADS 4.
3. A 1 x 1.2cm hypointense area at base of right transitional zone near the base of the seminal vesicle with surrounding ill-defined margin, PI-RADS 4.
4. No evidence of regional metastasis.
