42b - Answer: 62-year-old male presents with elevated PSA level measuring 7.08 ng/mL

HISTORY: 

This 62-year-old male presents with elevated PSA level measuring 7.08 ng/mL. History of negative biopsy three years ago.

PRIMARY FINDING: 

PI-RADS 2.

FINDINGS:

Prostate gland measures 4.3cm in CC, 3cm in AP and 5cm in RL dimensions. Calculated prostate volume is 42cc and PSA density 0.17. No suspicious mass like T2 hypointense region with substantive diffusion restriction or early arterial enhancement and washout to suggest malignant neoplastic process in the peripheral zone. Transitional zone hyperplasia is noted. A 1cm in diameter area of moderate to severe diffusion restriction is seen in the left midsegment transitional zone which does not correspond to a suspicious T2 hypointense region (PI-RADS 2). Anatomical prostate capsule, bilateral neurovascular bundles and seminal vesicles are clear. Retroprostatic angles are preserved. 

Urinary bladder is not full but unremarkable. 

No lymphadenopathy identified in the pelvis or inguinal regions.  

No abnormal osseous enhancement identified. Mild capsulitis of bilateral hip joints along with multiloculated cyst formations at the right hip joint anterior aspect which may represent paralabral cyst formations. 

CONCLUSION

1. PI-RADS 2, benign hyperplasia of the transitional zone. 

2. Mild capsulitis of bilateral hip joints along with multiloculated cyst formations at the right hip joint anterior aspect which may represent paralabral cyst formations.
