43b - Answer: 63-year-old male presents with elevated PSA

HISTORY: 

This 63-year-old male presents with elevated PSA.

PRIMARY FINDING: 

PI-RADS 2.

FINDINGS:

Prostate measures 6.5cm in transverse dimension, 4.5cm in AP dimension and 5cm in craniocaudal dimension with calculated ellipsoid volume 77mL.  

A 4mm hypointense nodule at lateral right apical peripheral zone with no restricted diffusion probably is an extruded hyperplastic nodule. No areas of restricted diffusion in the peripheral zones. Multiple encapsulated nodules in the transitional zones. No erased charcoal sign. Capsules intact. Neurovascular bundles intact. Seminal vesicles atrophic. 

No lymphadenopathy in the visualized pelvis. 

No focal suspicious osseous lesions. 

Right inguinal hernia with fat. 

Colonic diverticulosis. 

Nonmass-like lobular area of signal alteration deep to the right inguinal ligament probably due to previous intervention. 

CONCLUSION

1. No MRI evidence of aggressive prostate carcinoma. A 4mm hypointense nodule with no restricted diffusion at apical right peripheral zone probably an extruded hyperplastic nodule. PI-RADS 2. 

2. Benign prostatic hyperplasia.
