49b - Answer: 79-year-old male presents with elevated PSA level

HISTORY: 

This 79-year-old male presents with elevated PSA level which measures 5.92 nanogram per mL. History of biopsy which was negative.

PRIMARY FINDING: 

PI-RADS 2.

FINDINGS:

The prostate gland measures 7.4cm in CC, 7.5cm in RL and 6.7cm in AP dimensions. The calculated prostate volume is 195cc. PSA density measures 0.03. Transitional zone of the prostate gland demonstrates hyperplasia. No T2 hypointensity with diffusion restriction in the hyperplastic transitional zone or peripheral zone of the prostate gland to suggest malignant neoplastic process. Compression of the peripheral zone is noted. Anatomic prostate capsule, bilateral neurovascular bundles and seminal vesicles are within normal limits. 

Severe diverticular disease of the sigmoid colon is noted. 

Urinary bladder is not full. Enlarged median lobe of the prostate gland demonstrates urinary bladder floor indentation. Diffuse wall thickening of the prostate gland with pseudo-diverticulation is noted. 

No abnormal enhancement is present in the pelvis or prostate gland. No enlarged lymph nodes identified. 

CONCLUSION

1. PI-RADS 2 - Benign hyperplasia of the transitional zone of the prostate gland. 

2. Severe diverticular disease of the sigmoid colon.
