50b - Answer: 66-year-old male presents with elevated PSA

HISTORY: 

This 66-year-old male presents with elevated PSA.

(QUIZ ANSWER) MOST LIKELY FINDING: 

PI-RADS 4.

FINDINGS:

Prostate measures 4.5cm in transverse dimension, 3.7cm in AP dimension and 5cm in craniocaudal dimension. Calculated bullet volume of 54mL. 

5mm in maximum dimension hypointensity at posterolateral corner of the left apical peripheral zone. There is focal decreased signal intensity of ADC map in the region. Suggestion of arterial enhancement present. Anterior to the finding is a 4mm round hypointense nodule at lateral aspect of apical left peripheral zone with no definite restricted diffusion. Encapsulated primary glandular hyperplastic nodules in the transitional zones. No confluent hypointense mass. Capsule is intact. Neurovascular bundle unremarkable. Seminal vesicles unremarkable. 

No lymphadenopathy in the pelvis. No focal suspicious osseous lesions.

Left undescended testicle.

Penetrating right side 11 o'clock anal fissure. Question microcyst vs. microabscess 4mm. 

CONCLUSION: 

1. 5mm hypointensity with suggestion of restricted diffusion and enhancement at the posterolateral apical left peripheral zone. PI-RADS 4. 

2. A 4mm hypointense nodule at the lateral aspect of the left apical peripheral zone with no restricted diffusion probably extruded hyperplastic nodule. PI-RADS 3. 

3. Left undescended testicle. 

4. Penetrating right side 11 o'clock anal fissure. Question microcyst vs. microabscess 4mm. Symptoms?
