HISTORY: 

This 67-year-old male presents with elevated PSA of 8.9.

PRIMARY FINDING: 

PI-RAD 1 for peripheral zone.

FINDINGS:

The prostate is 5cm in transverse dimension, 3.5cm in AP dimension, and 4cm in craniocaudal dimension.

No areas of restricted diffusion in the peripheral zone.

Bilateral ill-defined hypointensity in the transitional zone with obscured margins.

The capsule is intact. The seminal vesicles are unremarkable. Neurovascular bundles are unremarkable. 

No lymphadenopathy in the pelvis. 

Visualized osseous structures normal in signal intensity. 

CONCLUSION

1. No areas of restricted diffusion in the peripheral zone, PI-RAD 1 for peripheral zone. 

2. Bilateral nonfocal ill-defined moderate heterogeneous hypointensity in the transitional zone, PI-RADS 2. 

3. Inspissated concretions in the left seminal vesicle consistent with chronic stenosis or obstruction of ducts thereof.
