HISTORY:
This 67-year-old male presents with enlarged prostate, elevated PSA, retention.
PRIMARY FINDING:
PI-RADS 2 of 5.
FINDINGS:
The prostate gland measures 7.6 cm in CC, 5.6 cm in AP and 6 cm in RL dimensions. Calculated prostate volume is 134 cc. Severely enlarged transitional zone demonstrates substantive indentation to the urinary bladder floor. No suspicious T2 hypointensity with substantive diffusion restriction and/or early arterial enhancement and washout to suggest malignant neoplastic process in the transitional or peripheral zones. Central zone is compressed. Anatomical prostate capsule, bilateral neurovascular bundles and seminal vesicles are clear.
Foley catheter is in place. Substantive indentation of the enlarged transitional zone to the urinary bladder floor along with slightly increased trabecular appearance of the bladder wall.
No enlarged lymph nodes in the pelvis or inguinal regions.
No free or loculated fluid collections.
Intrasubstance micro tear is noted in the right hamstring origin. Mild capsulitis of bilateral hip joints. Otherwise the visualized aspects of the musculoskeletal structures are unremarkable.
Bilateral fat containing inguinal hernias are present.
CONCLUSION:
1. PI-RADS 2 of 5, benign hyperplasia of the transitional zone.
2. Substantive indentation of the enlarged transitional zone to the urinary bladder floor along with slightly increased trabecular appearance of the bladder wall.
3. Bilateral fat containing inguinal hernias.
4. Small capsulitis of bilateral hip joints and intrasubstance micro tear of right hamstring origin.
