68b - Answer: 65-year-old male presents with elevated PSA level measuring 14.99ng/mL

HISTORY: 

This 65-year-old male presents with elevated PSA level measuring 14.99ng/mL. No history of biopsy or surgery.

PRIMARY FINDING: 

PI-RADS 2.

FINDINGS:

Prostate gland measures 7.9cm in CC, 5.7cm in AP and 6.3cm in RL dimensions. Calculated prostate volume is 148cc and PSA density is 0.10. 

Transitional zone hyperplasia is present. No suspicious T2 hypointense region with substantive deficient restriction in the transitional zone. Peripheral zone demonstrates compression due to enlarged transitional zone. An ectopic nodule is seen at the posterior peripheral zone of the right mid segment. No early or arterial enhancement demonstrating T2 mass-like hypointensity in the peripheral zone. Anatomical prostate capsule, bilateral neurovascular bundles and seminal vesicles are unremarkable. 

No enlarged lymph nodes identified in the pelvis. No inguinal lymphadenopathy. Small fat-containing left inguinal hernia is present. 

Urinary bladder is not full but severe indentation of the enlarged transitional zone to the urinary bladder floor is present. 

Visualized aspects of the musculoskeletal structures are unremarkable. 

CONCLUSION

PI-RADS 2, benign hyperplasia of the transitional zone. Substantive indentation to the urinary bladder floor with mild trabecular appearance of the bladder wall. 

ADDENDUM:

Additional diffusion-weighted images are provided. No diffusion-restricted lesion in the areas that were not included in the prior study.
