HISTORY:
This is a 61-year-old male. Evaluate prostate cancer.
PRIMARY FINDING:
PI-RADS 5.
FINDINGS:
The prostate measures 4.5cm in transverse dimension, 4cm in craniocaudad dimension and 3cm in AP dimension with calculated fluid volume of 35mL.
Approximately 3cm in maximum dimension hypointensity in the posterolateral aspect of the left peripheral zones from the apex to the base with restricted diffusion and early arterial enhancement. Thickening and mild irregularity of posterolateral capsule with loss of fat plane to the neurovascular bundle. Mild hyperplasia of the transitional zones. Seminal vesicles atrophic.
No lymphadenopathy in the visualized pelvis.
No focal suspicious osseous lesions.
CONCLUSION:
2cm in maximum dimension hypointensity with restricted diffusion at the posterolateral left peripheral zones from the apex to the base, PI-RADS 5. Thickening of posterolateral capsule and with loss of fat plane to the neurovascular bundle suspicious for transcapsular extension.
