HISTORY: 

This 42-year-old male presents with moderate to severe headaches, dizziness and lightheadedness.

(QUIZ ANSWER) IS A FINDING IN THIS CASE: 

Partial empty sella.

Using the diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.

FINDINGS: 

No Chiari malformation.

Partial empty sella. No optic nerve sheath dilatation.

Carotid and basilar artery flow voids are intact.

Subtle hyperintensity within the left gracile lobe of the cerebellum measuring approximately 1cm which is nonsiderotic on blood-sensitive images.

No diffusion restriction.

No transependymal spread of CSF or sulcal effacement.

No air-fluid levels in the paranasal sinuses.

Venous sinuses are patent.

Carotid and basilar artery flow voids are intact.

No extraaxial collections or hemorrhage.

Atlantodental interval is maintained.

Calvarium is intact.

No intraconal masses.

Cerebral hemispheres and deep nuclei without hemorrhage, mass or edema.

IMPRESSION: 

1cm left-sided well-circumscribed lesion within the left cerebellar hemisphere without surrounding edema or evidence of acute hemorrhage. Finding could represent capillary telangiectasia or other form of low-flow vascular anomaly. Differential could include low-grade neoplasm. Contrast-enhanced examination and serial imaging are recommended to monitor lesional behavior.

No acute parenchymal or extraaxial hemorrhage. No supportive findings of axonal injury on blood-sensitive images.

Partial empty sella without other supportive findings of intracranial hypertension. Eye examination to exclude papilledema is recommended in the setting headaches.

Venous sinuses are patent.

No air-fluid levels in the paranasal sinuses.

No Chiari malformation.
