<div>HISTORY:&#160;This 72-year-old female presents with history of soft tissue mass, ganglion, thumb pain; nodule tenderness; discolored nail.(QUIZ ANSWER) MOST LIKELY DIFFERENTIAL:&#160;Glomus tumor.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS:&#160;Tendons: Normal flexor and extensor tendons without tenosynovitis, tendinosis or tear.Ligaments: Normal collateral ligaments of the 1st IP, MCP and of the CMC.Osseous: No macrofracture, infiltrative or destructive lesion.&#160;Moderate osteoarthritic joint space loss at the 1st IP.&#160;Mild arthrosis of the 1st MCP and CMC.General: Multilobulated cystic lesion of the 1st nail bed, with T2 hyperintensity, T1 intermediate signal, at the nail bed matrix, with surrounding soft tissue swelling but no underlying bone edema.&#160;The lesion measures 5.5 x 1.0 x 0.4cm.CONCLUSION:1. Multilobulated cystic lesion at the matrix of the 1st nail bed, without underlying erosion, differential diagnostic considerations including glomus tumor, posttraumatic pseudocyst/ganglion.&#160;Consider ultrasound or contrast imaging to assess for solid or cystic nature.2. Moderate arthrosis of the 1st IP joint.</div>
