<div>HISTORY:&#160;This 46-year-old female reports with lump in the distal shin after falling in the shower and striking the shin.&#160;Nonpainful. Evaluate for fascial tear versus hematoma.(QUIZ ANSWER) NOT A LIKELY FINDING IN THIS CASE:&#160;Tibia bone fracture.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Sitting atop or superficial to the myofascial junction of the distal 3rd of the tibia is a fluid collection that is 2.3cm transverse x 7.4mm AP x 7.71cm long.Differential diagnosis lies between organizing hematoma that is very dilute and manifestation of fascial degloving so-called Morel-Lavallee syndrome.&#160;The surrounding soft tissues are swollen.Given the history, an infectious etiology is unlikely, however the surrounding soft tissues are diffusely indurated.No underlying fracture or bone injury is identified.Incidental views of the foot and ankle demonstrate a cyst arising from the subtalar space most likely a bursal cyst arising from the inferolateral subtalar retinaculum also known as a stem ligament, bursal cyst or fundiform ligament bursal cyst.&#160;Small erosions are noted at the ankle joint.CONCLUSION:Dilute hematoma versus fascial degloving syndrome.&#160;The collection, while small, is almost 7cm long and is surrounded by reactive inflammation and / or induration.</div>
