<div>HISTORY:&#160;This 65-year-old female reports with a history of giant cell tumor tendon sheath.(QUIZ ANSWER) MOST LIKELY DIFFERENTIAL:&#160;Pigmented villonodular synovitis (PVNS).Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Beginning just proximal to the MCP of the 1st digit and extending as far distally as the DIP (thin sessile component) is a soft tissue mass whose epicenter is at the level of the proximal phalanx. The masses AP cross-sectional diameter is 2.15cm. The bulk of the mass has a proximal to distal dimension of 1.83cm. While the mediolateral dimension is difficult to absolutely measure, the mass dissects underneath the flexor polices between it and the bone but also lies superficial to it and therefore wraps around it almost 360 degrees.&#160;No joint invasion, either IP or MCP is noted.&#160;While a differential diagnosis could be generated, the findings are consistent with the diagnosis of giant cell tumor of tendon sheath, so called localized PVNS.&#160;Finally, no bone invasion is present at this juncture.&#160;CONCLUSION:&#160;Large soft tissue mass with key geography as described in the body of the report consistent with giant cell tumor of tendon sheath.</div>
