<div>HISTORY:&#160;This 45-year-old male presents with soft tissue mass in left thigh for three months.(QUIZ ANSWER) MOST LIKELY PRIMARY FINDING:&#160;Malignant fibrous histiocytoma.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: 17cm x 7.2cm x 4.5cm heterogeneously enhancing solid mass within the posterolateral aspect of the vastus lateralis obliquus dissects anteriorly into the lateral aspect of the vastus intermedius and sits on the surface of the femur. No underlying femur signal abnormality to suggest its invasion. The lesion touches the posterolateral surface of the vastus lateralis obliquus, but does not enter the biceps femoris. There is a prominent feeding vessel at the posteromedial aspect of the mass that comes from the profunda femoris.&#160;Mild left hamstring origin tendinosis without macrotear.&#160;Small scrotal hydroceles.&#160;The posterior compartment is normal.&#160;No adenopathy in the thigh.&#160;CONCLUSION:&#160;Aggressive soft tissue mass in the vastus lateralis obliquus and the lateral periphery of the vastus intermedius, at the surface, but not invading the femur, most compatible with a malignant fibrous histiocytoma (pleomorphic undifferentiated sarcoma) with differential diagnostic considerations including less likely synovial sarcoma, myogenic sarcoma.</div>
