<div>HISTORY:&#160;This 22-year-old male presents with constant dull sore / swollen ring finger for eight months. There is swelling and discoloration with pressure.(QUIZ ANSWER) IS A DIFFERENTIAL DIAGNOSTIC CONSIDERATION IN THIS CASE:&#160;Both of the above should be considered.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: A bone marrow edema pattern involving the middle and distal phalanx of the ring finger is observed with marked adjacent soft tissue edema. The collateral ligaments appear intact. The flexor and extensor tendons appear intact. There is no evidence of extensor hood injury.&#160;IMPRESSION: Right Ring Finger: Extensive periarticular osseous reaction involving the distal interphalangeal joint of the ring finger with bone marrow edema, effusion and adjacent soft tissue edema. Differential diagnostic considerations include and favor septic arthritis with osteomyelitis and inflammatory arthritis with aseptic periarticular osseous inflammation.</div>
