Dr. P. here summarizing a few caveats that apply to PI-RADS 2.1 but also to PI-RADS 2.0. For the peripheral zone where most cancers occur, diffusion weighted imaging is the primary determining sequence. It's the dominant techniques. So if the DWI score is four and the T2 weighted score is only two, the PI-RADS assessment category should be a four. In other words, DWI wins.

In the TZ, it's very common to see one or innumerable diffusion restricting nodules in the TZ. For the TZ, the T2 weighted image is the primary determining sequence. What am I looking for there? Lentiform, ill define, smear type, erasure type, intermediate to charcoal like areas of hyper intensity that cross boundaries, disrespect nodules, disrespect the surgical capsule, disrespect the anatomic capsule, et cetera. So if the T2 weighted score is four and the DWI score is two, the PI-RADS assessment category should be a four. Let's move on to some additional caveats.
