Patient History
28-year-old male follow-up after opioid overdose

Findings
There is diffusion restriction in the bilateral globus pallidus and in the right frontal lobe. There is no intracranial hemorrhage.

Bilateral large mastoid effusions are seen, increased in size. The sphenoid sinus remains near complete opacified. There is stable mucosal thickening in the bilateral maxillary sinuses, with a new fluid level in the left.

Impressions
MRI brain demonstrates anoxic ischemic changes in the globus pallidus and lacunar infarcts in the right frontal lobe.
