Patient History
New right-sided visual neglect. Primary parkinsonism.
Findings
These images demonstrate ventricular dilatation and sulcal dilatation as expected for age. There is minimal periventricular white matter and subcortical white matter punctate high signal intensity. There is diminution in the size of the midbrain on the sagittal scan with widening of the interpeduncular cistern.
There is no evidence of restricted diffusion.
The patient shows complete opacification of the right maxillary antrum with polypoid change extending into the nasal cavity.
Impressions
Age-appropriate volume loss and small vessel ischemic change. Diminished size of the midbrain corresponds to clinical diagnosis of progressive supranuclear palsy with "hummingbird" sign.
