Patient History: 32 year old man with T wave inversion on ECG and LVH on echocardiography
Findings
Massive left ventricular hypertrophy asymmetric to the basal interventricular septum. Maximal measured wall thickness was 30mm.
Patchy areas of mid wall delayed enhancement in the left ventricle predominantly basal anterior septum at the junction points of the ventricles
Impressions
HCM . Asymmetrical septal variant. Massive hypertrophy 30mm. Extensive fibrosis > 15%.
