Patient History: 54-yo female with right hip pain
Findings
SKELETAL: No bone contusion, destructive bone lesion or macrofracture. Mild fibrovascular marrow response within the superolateral aspect of the femoral head at the femoral head-neck junction.
PELVIS: The pelvic viscera is normal. No free fluid. No hernia.
JOINTS: Thickened right acetabular labrum with mucoid degeneration and full thickness/near full thickness tear along the lateral and anterior aspect. Normal symphysis pubis and SI joints.
ACETABULUM/LABRUM: Lateral and anterior acetabular labral tear, with intralabral pseudocyst formation and/or mucoid degeneration. Chondromalacia of the acetabular roof, grade 2. Small erosions and pseudocysts anterolateral right acetabular rim. Acetabular undercoverage.
MUSCLES/TENDONS/LIGAMENTS: Abnormal signal within the ligamentum teres pubic and ischial components. Intact gluteus medius and minimus insertions. Normal hamstring origin. No ischiofemoral impingement. The pelvic and extrapelvic musculature is normal.
OTHER/SOFT TISSUE: No muscle atrophy or tear.
Conclusion
1. Lateral rim acetabular chondromalacia, small erosions and pseudocysts.
2. Lateral and anterior hypertrophic acetabular labral tear.
3. Degenerative midsubstance tear of the ligamentum teres. No AVN.
4. Acetabular undercoverage.
