<div>HISTORY:&#160;This 58-year-old male case is a followup for supratentorial tumor.(QUIZ ANSWER) WHICH SHOULD BE CONSIDERED?:&#160;All of the above.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.COMPARISON:Comparison&#160;MR from four months prior.FINDINGS:&#160;A left centrosylvian temporal lobe mass is identified lying deep and inferior to Broca's area and involving the left orbitofrontal gyrus.Subfrontal involvement includes the diagonal band of Broca.Focal area of encephalomalacia in the left periventricular region involving the upper anterior margin of the mass is consistent with a prior biopsy locus. A less likely consideration, although unchanged from four months prior, is an area of cyst formation along the anterior aspect of the mass.Overall, the contour, degree of mass effect, volume covered, and area covered from four months prior has remained stable. No new foci of displacement or herniation identified.Given the absence or paucity of enhancement after contrast administration, the differential diagnosis includes infiltrative astrocytoma, anaplastic astrocytoma, and less likely glioblastoma multiforme. In the absence of hemorrhage or calcification, oligodendroglioma is a consideration but also less likely.CONCLUSION:Left temporal lobe mass likely astrocytic or glial in nature, stable since MR of four months prior.</div>
