<div>HISTORY:&#160;This 28-year-old male presents with headaches, nausea and vomiting.(QUIZ ANSWER) FINDINGS ARE MOST CONSISTENT WITH:&#160;Tectal glioma.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Calvarium is intact.&#160;Craniocervical junction is without Chiari malformation.&#160;Partial empty sella.&#160;Bilateral optic nerve sheath dilatation and globe flattening.&#160;Lateral ventricular enlargement with enlargement of the 3rd ventricle and temporal horns bilaterally. There is decrease in the mamillopontine distance. Findings are consistent with obstructive hydrocephalus. The 4th ventricle may be mildly prominent.&#160;There is a 2.1 x 2.1cm subtly enhancing mass involving the quadrigeminal plate and left pulvinar, which is hyperintense on FLAIR and T2 images. No diffusion restriction on diffusion-weighted images.&#160;The 7th / 8th nerve complexes are intact. Cochlea and vestibule are intact. Otomastoid airspaces are clear.&#160;Cerebral hemispheres and deep nuclei are otherwise without hemorrhage, mass or edema.&#160;No parenchymal hyperintensity suggestive of neurofibromatosis. No evidence of acoustic neuromas.&#160;Masticator, parapharyngeal, pharyngeal mucosal, prevertebral and parotid spaces are without masses or adenopathy.&#160;No meningeal enhancement or evidence of basilar meningitis.&#160;No enhancement of the descending portion of the 7th nerve complexes.&#160;No para- or retrosellar masses or cysts.&#160;CONCLUSION:&#160;1. A 2.1cm x 2.1cm mildly enhancing mass involving the quadrigeminal plate and left thalamus resulting in obstructive hydrocephalus with enlargement of the lateral ventricles temporal horns and 3rd ventricle with decrease in the mamillopontine distance. 4th ventricle may be slightly prominent. Findings are most consistent with a glial neoplasm such as tectal glioma. Enhancement and pattern morphology appear less typical for germ cell tumor or primary pineal region lesion.&#160;2. Empty sella, globe flattening, optic nerve sheath dilatation are likely consequences of obstructive hydrocephalus and increased intracranial pressure.&#160;3. No other intracranial evidence of neurofibromatosis.&#160;</div>
