Case 15 - 56-year-old male presents with pain in the left groin

<div>HISTORY:&#160;This 56-year-old male presents with pain in the left groin.&#160;History of injury two years prior.PRIMARY FINDING:&#160;Osteoarthritis (OA).Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS:&#160;No recent macro fracture or contusion identified.Left hip: Shallow acetabular cup and wide femoral neck are consistent with adult type hip dysplasia.&#160;Femoral head demonstrates a sphericity with a large osseous bump at the head and neck junction which contributes to CAM type femoroacetabular impingement.&#160;Severe joint space narrowing and large spurring at the acetabulum are noted.&#160;Moderate to severe effusion is present.&#160;Large paralabral cyst formations versus capsular synovial cysts are seen at the lateral aspect of the femoroacetabular joint, which measures approximately 6 cm in AP 4.5 cm in CC and 1.5 cm in ML dimensions.&#160;Ligamentum teres and transverse ligament are intact.&#160;Iliofemoral and ischiofemoral ligaments are preserved.Mild peritendinitis of the gluteus minimus and left hamstring origin are noted.&#160;Otherwise the musculature and tendons about the left hip are unremarkable.&#160;A large fluid collection (12.5 cm in CC, 4 cm in AP and 2.5 cm in RL dimensions) containing multiple fat globules is seen superficial to the iliotibial band, consistent with Morel-Lavallee lesion.Right hip: Mild adult type hip dysplasia, and aspherical femoral head and osseous bump at the head and neck junction contributing to CAM type femoroacetabular impingement are noted.&#160;Associated degenerative arthrosis is less conspicuous compared to left and moderate in severity.&#160;Acetabular labrum is chronically torn.&#160;Subtle capsulitis is noted.&#160;Mild peritendinitis of the gluteus medius is seen.&#160;Mild ileo psoas bursitis is also present.Visualized aspects of the intrapelvic organs are unremarkable.No soft tissue mass identified.CONCLUSION:1. Advanced osteoarthrosis of the left hip joint which was previous pills by hip dysplasia and CAM type femoroacetabular impingement.2. Moderate effusion.&#160;Multiloculated fluid collection at the superolateral aspect of the hip joint, capsular synovial cyst is favored over paralabral cyst.3. Moderate degenerative arthrosis of the right hip joint, which is predisposed by the mild adult type hip dysplasia and CAM type femoroacetabular impingement.4. Morel-Lavallee lesion superficial to left iliotibial fascia.</div>
