Patient History
CMR images of a 60 year old patient with progressive shortness of breath are presented
Findings
There is atrioventricular discordance and ventriculoarterial discordance. The right atrium enters the left ventricle, which gives rise to the pulmonary artery, and the left atrium enters the right ventricle, which gives rise to the aorta. The morphological right ventricle with the moderator band is in the levoposition on the left side of the left ventricle. The aorta is anterior and to the left of the main pulmonary artery. Tricupsid valve insufficiency. The right ventricle has a depressed systolic function and is globally hypokinetic.
Impressions
Congenitally corrected Transposition of the great arteries ( CCTGA)
Systemic right ventricular dysfunction and tricuspid valve insufficiency.
