Patient History
An 18 year-old girl presents with fatigue and increasing shortness of breath on exertion.

Findings
There is apical displacement of the attachment of the septal leaflet of the tricuspid valve. The anterior leaflet of the tricuspid valve is elongated, sail like and tethered. Tricuspid regurgitation is present. A portion of the right ventricle is atrialized.
