In the imaging of bone tumors, making a distinction between a benign and malignant tumor can be difficult. Radiographs are often diagnostic, along with CT and MR, and pathologists and physicians often look to radiologists to determine a specific diagnosis. 

Join Dr. Gitanjali Bajaj (University of Arkansas for Medical Sciences) as she reviews various types of bone lesions and the discriminating features she looks for to make a specific diagnosis. Learn more about the most important considerations for evaluating these cases: 

  1. Age and location of tumor, which should prompt radiologists to work through a short list of differentials before looking at the lesion 
  2. Looking at the margins – does the lesion look aggressive (well circumcised) or non-aggressive (ill-defined)? 
  3. Taking into account the matrix – are we looking at a cyst, or does this entity have a ground glass matrix or matrix mineralization in the form of osteoid or chondroid matrix? 
  4. Looking for periosteal reaction – any surrounding bone marrow or soft tissue edema, joint effusion, or does the lesion cause cortical disruption and extend beyond bone into adjacent soft tissue?

Each case includes helpful breakdowns of MR, CT, and X-Ray findings, key teaching points, and some best practices for imaging rationale, indications, optimal imaging protocols, and reporting. 

Test yourself to determine:

  • If a complication associated with an entity is a malignant transformation, hematoma from pseudoaneurysm rupture, bursitis, or fracture and hematoma foundation?
  • Whether pain and discomfort in a leg can be diagnosed as benign enchondroma, low grade chondrosarcoma, bone infarct, or clear cell chondrosarcoma? 
  • In a case of elbow pain and swelling, what set of differentials is best to consider: Aneurysmal bone cyst (ABC)/Giant cell tumors (GCT), Chondroblastoma/GCT, ABC/Non ossifying fibroma, or Chondroblastoma/ABC
