Patient History: Patient fell 2 days ago with twisting injury to the foot


FINDINGS: There is soft tissue swelling with findings consistent with an acute Lisfranc fracture dislocation. There is mineralization within a widened proximal Lisfranc interval with lateral and dorsal translation of the base of the first metatarsal. Cortical avulsion fracture is present at the base of the second metatarsal, medially, with lateral translation of the lesser metatarsals with notable incongruity of the second TMT joint. There is a possible fracture of the distal third cuneiform.

IMPRESSION: Findings consistent with Lisfranc fracture dislocation with soft tissue swelling and incongruity of both the first and second TMT joints.
