Hypopharyngeal Carcinoma - T Staging

Today I'd like to discuss the staging of hypopharyngeal

malignancy according to the most recent AJCC

8th edition guidelines issued in early 2018.

Firstly, let's focus on T staging.

T staging follows the same principles as for the

oropharynx; however, there are individual anatomical

structures which must be recognized in

order to specifically stage, particularly when

we're dealing with a more advanced malignancy.

So T1 corresponds to a tumor which is

less than or equal to two centimeters and

limited to one subsite of the hypopharynx.

Remembering that the subsites are the piriform sinus, the

postcricoid region, and the posterior hypopharyngeal walls.

A T2 tumor is a tumor which measures greater than

2 centimeters, but less than or equal to 4 centimeters,

or alternatively involves more than one hypopharyngeal subsite,

and there must be no hemilaryngeal fixation.

A T3 tumor is a tumor that measures more than 4

centimeters, or displays hemilaryngeal fixation.

A T4a tumor invades one of the following structures:

a laryngeal cartilage, so thyroid, cricoid, or arytenoid;

the hyoid bone; the thyroid gland; esophageal muscle;

central compartment soft tissue, so that's soft tissue

surrounding the thyroid gland within the visceral space,

including prelaryngeal strap muscles and subcutaneous fat.

And then T4b disease is disease that invades

the prevertebral fascia, or encases the

carotid artery by more than 270 degrees,
