Hello everyone, it's Dr. Sidney Levy here and today we will be
0:02
3 00:00:05,020 --> 00:00:08,060 talking about lymph node levels of the neck.
0:08
Lymph node levels of the neck are an important
0:11
fundamental piece of knowledge in order to
0:13
be able to stage head and neck malignancy.
0:16
And I would like to take you through each of the
0:19
accepted lymph node levels and draw them for you on
0:23
MRI images so that you can quickly identify them.
0:28
The first vignette will be on Level 1, which is
0:31
the submental and submandibular lymph node group.
0:36
Level 1 is split into two
0:40
sublevels, Level 1A and Level 1B.
0:43
Level 1A is the submental group, and
0:45
Level 1B is the submandibular group.
0:49
I will use these pre-contrast T2 and T1-weighted
0:53
images to demonstrate the boundaries for you.
0:57
Firstly, Level 1A.
1:00
The structures that you need to be familiar with are
1:04
the anterior belly of the digastric and the hyoid bone.
1:10
On the axial projection, the anterior belly of
1:14
the digastric is found here and here, so these two muscles.
1:23
It is often difficult to locate them on a
1:25
single slice, and because of that, I suggest
1:30
that you become familiar with scrolling from
1:34
the origin of the posterior belly of the digastric
1:40
inferiorly into the anterior belly, where
1:45
it will eventually insert on the hyoid bone.
1:49
The hyoid bone is the next structure
1:51
that's important, and that's here.
1:55
It can be located on any plane, but sometimes,
1:59
it's easy to get an appreciation of the
2:01
vertical level by using the sagittal plane.
2:04
And an easy way to find it is that many of the
2:08
extrinsic muscles of the tongue actually insert on it.
2:12
We have the geniohyoid, for instance.
2:18
And once you're comfortable with those structures, what
2:21
you need to remember is that the submental level is
2:26
anterior and medial to the anterior belly of the digastric.
2:32
So, to reinforce what I've been talking
2:37
about, we can consider the submental level
2:42
to be in this region here, remembering that
2:47
the anterior belly of the digastric is here.
2:51
In a vertical plane, you just need to remember that
2:55
it is superior to the inferior margin of the hyoid.
2:59
So, therefore, you are looking
3:04
at lymph nodes in this region.
3:06
Now, onto Level 1B.
3:08
Level 1B is the submandibular lymph node group.
3:12
And the first thing to remember is that it is lateral,
3:17
and posterior to the submental group.
3:22
Its important landmarks are very similar.
3:26
Once again, you have the anterior belly of the digastric.
3:31
But this time, you have to designate the
3:33
boundary between Level 1B and Level 2A.
3:38
And the important landmark to remember is the
3:40
posterior border of the submandibular gland.
3:44
So, if you draw a plane here to remember
3:48
where our anterior belly of the digastric is,
3:51
the submandibular, or Level 1B level, is lateral
4:00
to the anterior belly of the digastric and anterior
4:05
to the posterior border of the submandibular gland.
4:09
So, if I scroll for you,
4:12
you will often locate lymph nodes
4:15
in close proximity to the
4:16
inferior margin of the mandible.
4:19
But this is the region that we're concerned with.
4:23
In our next vignette, we will discuss Level 2A
4:27
and Level 2B, which are posterior to Level 1B.
