The nodal pattern of drainage of soft palate

squamous cell carcinoma tends to be to the

ipsilateral or sometimes contralateral level 2 lymph

nodes due to their proximity to the soft palate.

The only other thing that's important to remember

is that given its location, retropharyngeal lymph

nodes can also be involved by soft palate tumors.

So it's important to look in the region where

retropharyngeal lymph nodes tend to be located,

as occasionally you will find an abnormally

enlarged asymmetric retropharyngeal lymph node.

So, in summary, soft palate tumors have a tendency

to spread infralaterally to the palatine tonsil, or

across the midline to the contralateral soft palate.

But it's also worth remembering that sometimes

they may spread to involve the nasopharynx

superiorly, the hard palate anteriorly, or

infiltrate parapharyngeal fat laterally.

Their nodal drainage tends to be to level 2

on the ipsilateral or contralateral side.

But remember, there can also be

retropharyngeal lymph node involvement.

And it's very important to

localize the tumor effectively.

So that you're confident it's a soft palate

tumor as opposed to a palatine tonsil tumor.

And keep in the back of your mind that

there are other diagnoses such as radiation

change or minor salivary gland malignancy.
