Female Pelvis Anatomy on CT

0:00

This is a contrast-enhanced

0:02

CT of the female pelvis.

0:04

And let's review some of that normal

0:06

anatomy that we just talked about.

0:08

So here we are on the axial view

0:10

at the level of the pelvic inlet.

0:12

We can see the iliac bones out here.

0:15

And as we scroll down, we start

0:17

to see the uterus come into view.

0:19

Now it's important to remember that

0:20

it can be challenging to identify the

0:22

uterus and ovaries on CT, especially if

0:25

there's a lot of bowel in the region.

0:27

But again, we see the myometrium,

0:29

and this is the endometrial cavity.

0:31

As we scroll down, we start to

0:33

see the ovaries come into view.

0:35

This is the right ovary.

0:36

We can see that it has a peripherally

0:38

enhancing structure within it.

0:40

This is a corpus luteum.

0:42

And then we also see the left ovary over here.

0:46

The ovaries on CT will look like

0:48

hypodense, almond-shaped structures,

0:51

again, typically lateral to the uterus.

0:54

This is a sagittal reconstruction on the CT,

0:58

which is ideal for evaluating the uterus.

1:01

So again, here's the uterine fundus.

1:03

This is the body.

1:04

We see the myometrium and endometrial cavity.

1:07

This is the cervix, anterior

1:09

lip, and posterior lip.

1:10

This patient has a small

1:12

exophytic uterine fibroid as well.

1:15

The coronal view can be helpful in identifying

1:18

the ovaries relative to the uterus.

1:21

So this is a coronal reconstruction of the CT.

1:24

We are looking down the endometrial

1:26

cavity, which is here centrally.

1:27

This is myometrium surrounding

1:29

the endometrial cavity.

1:31

And out laterally, we start to see the right

1:33

ovarian vascular pedicle and right ovary.

1:36

And so again, here's the right ovary.

1:38

Here's that corpus luteum.

1:40

We can see it as sort of an

1:41

extension coming off of the uterus.

1:43

And here is the left ovary, which again can

1:47

appear as a hypodense almond-shaped structure.

1:51

When you're attempting to identify the

1:53

ovaries on CT, it can be extremely helpful

1:56

to also identify the gonadal veins.

1:59

And so this is the right gonadal

2:00

vein, and if we follow it, we can see

2:02

that it connects to the right ovary.

2:05

So that confirms for us that we have

2:07

correctly identified the ovary on CT.
