I'd like to show this next case of bilateral

Here we have, on the right side, a patient who has

an area of demineralized bone in the fissula ante fenestram,

anterior to the vestibule and oval window.

And we have just a faint view of the stapes' footplate, here.

Again, we use the term otospongiosis instead

of its synonym otosclerosis,

to emphasize that this is spongiotic demineralized bone,

not sclerotic hyperdense bone.

This patient had conductive hearing loss,

secondary to the fixation of the stapes.

What is the recourse?

Well, as you can see on this left hand side,

which was the more affected side,

the patient has a structure which

is leading to the oval window,

which appears more dense than one would

expect for a normal stapes.

Here is the vestibule with the oval window.

Here is this abnormal structure.

And what this represents is what is known as a PORP.

It's a Partial Ossicular Replacement Prosthesis.

This is a prosthesis for the stapes,

and it's a single piston prosthesis that will communicate

with the incus and lead to improved transmission of the

vibrations of sound into the perilymph and the endolymph.

So, this is one of the ways that one can treat patients

who have otospongiosis with a stapes prosthesis,

if the issue is purely fixation of the stapes.

Let me just point out the spongiotic bone

is a little bit more lucent, right here,

this is how we make the diagnosis of otospongiosis.
