Now here's a pedunculated

mass, but it's not a type 4.

It's an example of balanced hypertrophy.

In other words, here's the

urethra coming down like this.

There's a little slit of it there.

Behind the urethra, there's enlargement.

And in front of the urethra, there's enlargement.

So this one's relatively balanced.

But the posterior superior component has

a neck that's narrower than the apex.

So it's starting to pinch off,
