This is an MRI of the brain and face in a one
0:05
month old with an asymmetric growth on the
0:10
left side of the face, left cheek region.
0:13
It was thought to be a mass, but if you
0:15
look at it, on this T2-weighted image, it's
0:18
relatively homogeneously hyperintense.
0:22
It's hyperintense on T1-weighted
0:24
imaging, suppresses on fat-suppressed
0:28
T1-weighted imaging, and There's no
0:32
suspicious enhancement within it.
0:35
So this looks at first like a
0:38
lipoma, which is a benign entity.
0:43
Obviously, there may be cosmetic
0:44
implications, but it's a benign entity.
0:47
There was more to it than that.
0:50
If I go back to the T2-aided image, if
0:52
I look at the cerebral hemispheres, The
0:55
right cerebral hemisphere protrudes
0:57
a little bit over to the left of midline.
0:59
So if you actually measure it,
1:01
the right cerebral hemisphere is
1:02
a little bit larger than the left.
1:04
The right cerebral hemisphere, look at the
1:07
T2-weighted image of the white matter, it's
1:09
a little bit more hypointense than the left.
1:13
And Additionally, it's a little bit more
1:16
hyperintense on T1-weighted imaging than
1:18
the left cerebral hemisphere white matter.
1:21
And it's noted that in a one-month-old,
1:23
this T2 hyperintense, T1 hyperintense
1:26
appearance of the white matter is normal.
1:28
So while anytime you see an asymmetry, it's hard
1:31
to know what's abnormal, the left hemisphere
1:35
seems to have a normal myelination pattern.
1:38
Now, if I go up more superiorly, I'm
1:41
seeing irregular sulcation pattern.
1:44
So this patient has what
1:47
looks like polymicrogyria.
1:50
And again, this hemisphere looks a tiny
1:53
bit larger, and while there's a little
1:56
bit more hypointense appearance on T2 and
1:59
hyperintense appearance on T1-weighted
2:00
imaging of the white matter, that can
2:02
be seen with accelerated myelination.
2:05
Well, accelerated myelination and
2:09
asymmetrically enlarged hemisphere,
2:11
or portions of a hemisphere.
2:13
is a finding that can be seen with hemimegaly
2:16
encephaly now hemimegaly encephaly does not even
2:18
have to involve the whole hemisphere, it can
2:20
be subhemispheric hemimegaly encephaly, then
2:24
this patient has polymicrogyria, which also
2:26
can be associated with hemimegaly encephaly.
2:30
So let's take a look again at the cheek mass.
2:34
So, a biopsy of this.
2:37
showed that there was a
2:38
mutation in the PIK3CA gene.
2:42
What does that mean?
2:43
Well, the PIK3CA gene is noted to be associated
2:48
with a rare congenital overgrowth syndrome
2:51
known as Clove's syndrome, C L O V E S.
2:55
That is an acronym for, you know,
2:58
congenital lipomatous overgrowth.
3:00
There's vascular malformations, epidermal
3:02
nevi, and skeletal abnormalities.
3:05
So this lipomatous overgrowth here of
3:10
the cheek is actually related to Clove
3:13
syndrome with this PIK3CA mutation.
3:18
Now we just went through that acronym
3:21
and I don't see anything necessarily
3:24
about the brain malformation, except
3:30
remember one of them is overgrowth.
3:32
And you could say that there's overgrowth
3:34
of the brain, but it's noted that
3:37
the PIK3CA mutation is actually a
3:41
different mutation in the PIK3CA gene.
3:44
is associated with hemimegaly encephaly.
3:46
So it's not unexpected that someone with
3:49
Clove syndrome may have hemimegaly encephaly.
3:52
It just happens to be contralateral to
3:54
the lipomatous overgrowth on the face.
3:57
So this patient has a facial lipomatous
4:03
overgrowth, right-sided hemimegaly
4:07
with polymicrogyria and accelerated
4:09
myelination in the setting of Clove syndrome.
