and someone in a prior MRI and a prior report said

that there was an adenoma in this gland.

Now, this being a man,

we know that prolactin secreting adenomas are

more common in women by about five to one.

The growth hormone secreting ones more

common in men by about two to one.

And the ACTH type really important with

with a slight female predominance.

So does this patient have a microadinoma or an adenoma?

Well, this is one where some of the anatomy that Dr.

so the stars of the show are kind of like the anterior

lobe of the pituitary, the adeno hypothesis.

Here's your bright spot, the neuro hypothesis.

But there's actually a little zone in there

which is the pars intermedia just part of

the anterior gland. That's correct.

It's between the two posterior portion of

the anterior segment of the gland.

so there's a little segment right in.

Here. And so when we're looking for an adenoma,

you really expect it to be sort of in the

anterior portion of the gland.

So this lesion was being called an adenoma,

but it's really pretty far back,

right behind the infantibulum,

and actually right where you'd think the pars intermediate

would be. And then if you correlate on your images,

here's your T two. And so it's sort of cystic.

So I ended up thinking that this was probably a pars

intermediate cyst and that maybe the adenoma wasn't there.

And this patient actually does not

have endocrine disturbance. Okay.

The symptoms are something other than that.

Yeah, I read the history.

The history is kind of weird.

It really didn't point in any direction to a microadnoma.

Was, like,

headaches and memory loss and things of that nature.

and on the FLAIR, you can see the lesion is a little low.

On the T1, it's clearly low.

And on the axial T2, it is.

High in signal intensity.

You don't usually see untreated microadenomas

that are this bright.

So not only does the position go against a microadenoma,

the history goes against it, the gender goes against it,

and the T2 relaxivity goes against it.

Pituitary cyst or pars intermedia cyst is what it is.

So knowing where you are in the gland on that

anterior-posterior can be really important.

And knowing the segments of the gland where

things can come from is really helpful.
