<div>HISTORY:&#160;This 64-year-old female presents with right renal mass and liver cysts.(QUIZ ANSWER) REGARDING THE MASS IN THIS CASE:&#160;All of the aboveUsing the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS:&#160;As seen on recent MRI lumbar spine a 5.4 x 5.1 by 5.4 cm well encapsulated right posterior mid to lower pole renal mass is present.&#160;Mass is T1 predominantly dark signal, T2 predominantly bright signal with some T2 dark linear areas throughout.&#160;Following IV contrast, intense enhancement except for stellate central area as present.&#160;Mass extends to inferior aspect of right renal pelvis.&#160;The differential would include oncocytoma versus chromophobe renal cell carcinoma.&#160;Right kidney partially non rotated no obvious involvement of the right renal vein.No other renal mass or hydronephrosis present.Multiple hepatic cysts present the largest 2.3 x 1.6 cm in the posterior right lobe and 3.3 x 3 cm cyst from the posterolateral left lobe. No enhancing liver lesions.The gallbladder and bile ducts unremarkable.A 6 x 4 and adjacent 4 x 3 mm distal tail pancreatic cystic foci visualized.&#160;These appear to be related to pancreatic duct and do not demonstrate substantial enhancement.&#160;Pseudocysts, side branch clubbing or IPMN favored over other etiologies.&#160;Additional septated or two adjacent 3 mm cysts present in the upper distal body of the pancreas.Adrenal glands not remarkable.&#160;No bulky retroperitoneal adenopathy.No bowel obstruction.Severe lumbar spondylosis and mild to moderate levoscoliosis present.CONCLUSION:1. 5.4 x 5.1 x 5.4 cm well encapsulated enhancing right renal mass with central stellate necrosis.&#160;While this could represent oncocytoma, a chromophobe renal cell carcinoma is not excluded.&#160;Biopsy should be considered.2. Subcentimeter cystic foci in the pancreas which appear to be related to pancreatic duct and are favored to represent pseudocysts or other nonaggressive cysts.&#160;Six-month follow-up would ensure stability.3. Hepatic cysts.</div>
