<div>HISTORY:&#160;This 47-year-old male presents with right renal mass and left adrenal nodule.(QUIZ ANSWER) IN THIS CASE, WHICH IS CORRECT?:&#160;There is invasion of the right renal veinUsing the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Liver is normal in contour. No focal lesions in the visualized segments. No intrahepatic biliary dilatation. No gallstones.&#160;Pancreas unremarkable. No focal lesions. No ductal dilatation.&#160;&#160;&#160;Spleen normal in size.&#160;&#160;A 1cm left adrenal nodule; no signal drop on the out-of-phase images. No nodule in the right adrenal gland.&#160;An enlarged irregular right renal mass progressive in appearance measuring approximately 6.5 x 7.5 x 7.5cm with heterogeneous enhancement. The right renal vein is enlarged with heterogeneous enhancement consistent with tumor invasion. There are a few collateral veins around the right kidney.&#160;&#160;No lymphadenopathy in the visualized abdomen.&#160;&#160;No abnormal signal intensity in the visualized osseous structures.&#160;&#160;CONCLUSION:&#160;&#160;1. Large right renal mass with invasion of right renal vein suspicious for obstruction. A few collateral veins around the right kidney.&#160;&#160;2. A 1cm left adrenal nodule with no evidence of microscopic fat, fat-poor adenoma, pheochromocytoma, or metastasis.ADDENDUM:&#160;Review by this reader demonstrates a round, smooth, encapsulated 1.3cm left adrenal lesion.&#160;Although the lesion is fat-poor, the findings are consistent with and overwhelmingly favor a small adrenal adenoma and no further intervention other than observation is needed.&#160;&#160;CONCLUSION:&#160;Left adrenal adenoma.</div>
