<div>HISTORY:&#160;This 58-year-old female presents with mass in the right index finger large enough to weaken the bone.(QUIZ ANSWER) MOST LIKELY PRIMARY FINDING IN THIS CASE:&#160;Enchondroma.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: An expansile STIR bright, T2 intermediate, enhancing mass is seen expanding the proximal 2nd phalanx measuring in size 2.7cm longitudinal x 0.9cm AP x 1.2cm transverse. This lesion extends from the proximal epiphysis to the mid shaft of the phalanx. Associated cortical thinning is noted radial greater than ulnar. Minor lobulation of the distal edge is present. No extraosseous soft tissue mass is seen. No cortical break is visualized.&#160;Extensor and flexor tendons unremarkable.CONCLUSION:&#160;Expansile, lytic enhancing lesion within the proximal aspect of the 2nd proximal phalanx representing either an enchondroma or even possibly a giant cell tumor in light of the extension to the epiphysis proximally. Cortical thinning without evidence of a cortical break.PATH PROVEN: Enchondroma</div>
