Base of Tongue Oropharyngeal Carcinoma, N & M Staging

0:01

Hello everyone, Sidney Levy here continuing

0:03

our staging of this base of tongue malignancy.

0:06

Now the N staging.

0:08

This tumor has a single right ipsilateral

0:12

level 2a lymph node, which we can observe as a

0:18

heterogeneous enhancing enlarged lymph node here.

0:22

This lymph node measures less than three

0:25

centimeters and displays a smooth,

0:30

well-circumscribed margin with no suggestion of

0:33

irregular infiltration of surrounding soft tissue.

0:36

So therefore, according to our N staging, if this tumor is

0:42

found to be a human papillomavirus-negative tumor, this is

0:47

an N1 tumor with a single ipsilateral lymph node measuring

0:52

less than 3 cm with no evidence of extranodal extension.

0:58

If, however, the tumor is found to be human

1:02

papillomavirus-positive, it still remains an N1 tumor in

1:07

this particular patient because we have an ipsilateral

1:12

lymph node that measures less than 6 centimeters.

1:16

This is an M0 tumor because no evidence of distant

1:19

disease was found within the head and neck, but be

1:22

mindful that we need other imaging to confirm this.

1:26

Thank you.

1:26

Usually in the form of either CT or PET CT imaging.

1:32

So in summary, this base of tongue malignancy, which

1:37

began on the right but has crossed over to the left, is

1:40

invading the genioglossus, extrinsic tongue musculature,

1:45

and has a single ipsilateral lymph node measuring less

1:50

than 3 cm without evidence of extranodal extension.

1:53

It is therefore a T4A

1:56

N1M0 tumor and it remains N1 irrespective

2:02

of whether the tumor is later found to be

2:05

human papillomavirus-positive or negative.
