Metastases (pancreatic primary)

So here we have a 66-year-old female

with abdominal pain, and an MRI was obtained.

I want to start off by showing the MRCP images.

And so if you look at the MRCP images, there's

a lot going on, but just sort of focusing

on portions of the liver that we do see.

And we'll just zoom up on some of these images.

You can see that duct over here is

dilated and then abruptly narrows.

There's a duct over here that's

dilated and abruptly narrows.

There's a duct over here that's

And so you can see that over

here, a little bit over here.

And so kind of taking this all in,

there are really segmental regions of

biliary ductal dilatation in this liver.

And again, one of the entities

that often manifests with areas of

segmental biliary ductal dilatation

is primary sclerosing cholangitis.

If this is all you saw, you know, as a

case, you may think, "Oh, could this be a

case of primary sclerosing cholangitis?"

But obviously, you know, you

don't view images in isolation.

We'll look at everything else.

And so let's now go back to

some of our other sequences.

This is the T2-weighted image.

And we're certainly seeing

ducts that are dilated, right?

They're segmental ductal dilatation.

But in this instance, it's not really

strictures of the bile duct that's

causing it, but rather this is a liver

that contains numerous metastases.

And these are causing mass effect

on some of these bile ducts.

You can see this duct is dilated due to

a large metastasis over here and really

numerous metastases in this liver that

are resulting in multifocal regions

of intrahepatic ductal dilatation.

If you look even closer, you can see the

pancreatic duct is kind of dilated over

here, and there's a pancreatic neoplasm here.

So this turned out to be a pancreatic neoplasm,

adenocarcinoma with numerous liver metastases,

resulting in multifocal segmental regions

of intrahepatic ductal dilatation, a

finding which we often use to describe

1:58

primary sclerosing cholangitis, but

obviously can also occur with any entity

that causes mass effect upon the bile ducts.

In this instance, it was mass effect

from numerous liver metastases.
