Tubal Ectopic Pregnancy

HIDE
PrevNext

0:01

This is an ultrasound of the pelvis performed

0:03

in a patient presenting with exquisite right

0:06

pelvic pain and an abnormal beta HCG in the

0:09

context of a positive home pregnancy test.

0:12

We begin with transabdominal

0:14

images of the pelvis where the

0:15

sonographer has measured the uterus.

0:18

We can see the hyperechoic endometrial stripe

0:21

without a discrete intrauterine gestational sac.

0:25

Again, we have a coronal image of the uterus

0:27

where we can see the hyperechoic endometrium

0:30

and no apparent gestational sac visible.

0:33

In this patient with suspected ectopic

0:35

pregnancy, the sonographer has also obtained

0:37

images of the upper abdominal quadrants,

0:40

again to determine whether there is a

0:42

significant amount of intraperitoneal free fluid.

0:46

In this patient, we see a clear delineation

0:49

between the liver and kidney with intervening

0:51

fat and without significant free fluid present.

0:55

These are transvaginal images of the pelvis.

0:58

Again, we see the uterus.

1:00

With myometrium and echogenic endometrium, we

1:05

do not see an intrauterine gestational sac.

1:08

Lateral to the uterus in the right

1:10

adnexa, we see a peripherally hyperechoic

1:13

structure with central hypoechogenicity

1:17

and a well-circumscribed cystic structure.

1:21

A fetal pole is identified within this

1:23

structure in the right adnexa with a crown

1:26

rump length corresponding to an estimated

1:28

gestational age of 7 weeks and 1 day.

1:32

The yolk sac is also present within

1:34

this extrauterine gestational sac.

1:37

M-mode evaluation of the fetal pole demonstrates

1:40

positive embryonic cardiac activity with

1:42

a heart rate of 124 beats per minute.

1:46

The right ovary is identified and is separate

1:49

from the previously described adnexal lesion.

1:53

The left ovary is also identified

1:55

and is normal in appearance.

1:57

To summarize, we see a well-circumscribed,

2:00

hyperechoic structure that is separate

2:02

from the ovary in the right adnexa,

2:04

with internal hypoechogenicity and a visible

2:07

fetal pole with positive cardiac activity.

2:10

This is a viable ectopic pregnancy

2:14

within the right fallopian tube.
